CareLuxe Solutions LLC provides professional home care services in Pittsburgh, Pennsylvania and surrounding counties. This notice describes health-related information we may collect, why we collect it, how it is used, and your rights.
This notice supplements our general Privacy Policy and applies to health-related information collected through public website inquiries and pre-service conversations that may fall outside HIPAA.
What Consumer Health Data Means
Consumer health data may include information related to functional limitations, care needs, caregiver or care recipient status, interest in home care services, and insurance coverage type as it relates to care eligibility.
How We Use Consumer Health Data
We use this information to evaluate service needs, communicate about care, determine potential eligibility for Medicaid waiver programs, respond to inquiries, and improve service quality and safety.
How We Share Consumer Health Data
We share consumer health data only with caregivers, care coordinators, government agencies when required, trusted service providers under confidentiality obligations, with your consent, or as required by law.
